To make a good questionnaire you should start with basic details and move on to more difficult questions. Ensure that you use appropriate language to the people answering the questions, make it simple for them. Try to avoid questions that need personal or invasive answers. Depending on if you want specific answers or just a general conclusion, you can use open questions or closed questions. An open question allows people to write anything they want where as a close question restricts them by giving them options. It is important that your answers are honest, easy to understand and unbiased.
